Job summary

AWARE is seeking a dedicated targeted case manager to join our team in Montana. We provide care to adults, children, and families, focusing on compassionate, unconditional support and personal growth.

Responsibilities include managing case loads, helping clients access medical, social, educational, vocational, and recreational services, and coordinating billing on a monthly, quarterly, and yearly basis. Salary ranges from $20.00 to $22.19 per hour.

Qualifications

• BS/BA in a social work related field and at least one year of direct work experience in the developmental disability field.
• Ability to develop and implement individual treatment plans with knowledge of behavioral, clinical, mental health interventions, and skill-building techniques.
• Understanding of the emotional and behavioral needs of assigned clients.
• Interpersonal communications skills to work independently and as a team, observe and report accurately, and follow oral and written instructions.
• Ability to facilitate meetings.
• Ability to work independently via a virtual office using a laptop and cell phone.

Responsibilities

• Provide case management services to individuals who meet the definition of being developmentally disabled, as defined by the State of Montana.
• Assist these individuals in gaining access to medical services, social services, educational services, vocational services, and recreational services.
• Manage caseload and/or billing requirements on a monthly, quarterly, and yearly basis.
