Job summary

AWARE in Polson, MT is hiring a Targeted Case Manager. This full-time position offers $22.00–$27.33 per hour to start and focuses on helping adults, children, and families access medical, social, educational, vocational, and recreational services.

Ideal candidates hold a BS/BA in a social work related field with at least one year of direct developmental disability experience and can work independently in a virtual office using a laptop and cell phone.

Qualifications

• BS/BA in a social work related field required.
• Minimum of 1 year direct work experience in developmental disability field.
• Ability to develop and implement individual treatment plans with behavioral, clinical, and mental health interventions.

Responsibilities

• Provides case management services to individuals with developmental disabilities as defined by Montana.
• Help clients access Medical, Social, Educational, Vocational, and Recreational services.
• Manage caseload and billing requirements monthly, quarterly, and yearly.

Responsibilities:

• Provides case management services to individuals who meet the definition of being developmentally disabled, as defined by the State of Montana.
• Assist these persons gain access to the following:
• Medical Services
• Social Services
• Educational Services
• Vocational Services
• Recreational Services
• Caseload and/or billing requirements on a monthly, quarterly, and yearly basis.

Requirements

Talents, skills, and abilities:

• A BS/BA Degree in a social work related field; and minimum of one (1) year of direct work experience in the developmental disability field.
• Ability to develop and implement individual treatment plans; with the knowledge of behavioral, clinical, and mental health interventions, and skill building techniques.
• Understanding of the emotional and behavioral needs of assigned clients.
• Interpersonal communications skills necessary to; work independently and as a team member, make observations and to report them accurately, and follow oral and written instructions.
• The ability to facilitate meetings.
• The ability to work independently via a virtual office; using a lap top computer, and cell phone.
