Job summary

The Integrated Care Clinical Manager - Massachusetts is a community based registered nurse responsible for monitoring, follow-up and clinical care management for dually-eligible enrollees with complex medical, behavioral and social needs. The role coordinates care with providers and community resources to improve health outcomes and quality of life.

Responsibilities include leading the ICT, conducting assessments, and ensuring continuity of care through transitions, while complying with NCQA

Qualifications

• ASN in nursing from an accredited program
• Registered Nurse with ability to independently serve people with complex needs
• Prior care coordination/case management experience preferred

Responsibilities

• Engage with enrollees in their homes and community settings to establish a complex care management relationship
• Coordinate between healthcare providers, community resources, and enrollees for seamless care transitions
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs
• Oversee enrollee utilization of long-term services and supports and ensure access to resources
