Job summary

The Integrated Care Clinical Manager - Massachusetts at Commonwealth Care Alliance is a community-based registered nurse responsible for monitoring, follow-up and clinical care management for dually eligible enrollees with complex medical, behavioral and social needs.

This role focuses on integrating health services and community resources to coordinate enrollee care, improve health outcomes and enhance quality of life while collaborating with an interdisciplinary team.

Qualifications

• A.S. degree in nursing from an accredited program.
• Current RN license in Massachusetts.
• Experience in care coordination or case management preferred.

Responsibilities

• Engage with enrollees in homes and community settings to establish complex care management relationships.
• Lead the interdisciplinary care team (ICT) to create holistic care plans addressing medical and social needs.
• Coordinate with providers, community resources, and state agencies to avoid service duplication.
