Job summary

Commonwealth Care Alliance seeks an Integrated Care Clinical Manager - BH in Massachusetts to coordinate and manage care for dually eligible enrollees with complex needs. The role focuses on integrating health services and community resources to improve outcomes and quality of life.

The position requires leading an interdisciplinary team, organizing care plans, and ensuring effective communication among providers, community partners, and enrollees. Travel across communities will be required.

Qualifications

• Master's degree in social work or mental health counseling or equivalent licensure.
• Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) required.
• Experience with dual-eligible populations and care coordination is preferred.
• Medicaid/Medicare managed care experience is preferred.

Responsibilities

• Engage enrollees in their homes and community settings to build complex care management relationships.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and social needs.
• Coordinate with providers, community resources, and state agencies to avoid service duplication.
• Educate enrollees about benefits, services, and self-management of chronic conditions.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
• Advocate for enrollee needs within the healthcare system.
