Job summary

Qualifications

• Associates of Science in nursing required.
• Registered Nurse with ability to serve people with complex needs.
• Care coordination or dual-eligible population experience preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build care management relations.
• Lead the interdisciplinary care team to create holistic care plans.
• Coordinate with providers and community resources to avoid care duplication.
• Perform comprehensive and functional assessments and follow-up.
