Job summary

Father Bill’s & MainSpring is seeking a Regular Full Time Hourly Case Manager in Plymouth, MA. This Housing Department role focuses on providing community-based case management with a housing-first approach to help individuals and families achieve positive tenancies.

The position requires a Bachelor’s Degree or relevant experience; Master’s Degree is preferred, along with CPR certification. AED/First Aid may also be required.

Qualifications

• Bachelor’s Degree and/or relevant experience is required; Master’s Degree in social work or related field preferred and/or 2+ years direct care experience in related field.
• CPR certification is required and provided by FBMS; AED and First Aid may also be required and provided by FBMS.

Responsibilities

• Provide outreach and assessment in the community as deemed necessary by supervisor or administrator.
• Evaluation and assessment of referrals, utilizing a housing first framework and understanding the barriers to house people experiencing homelessness.
• Collaborate with FBMS shelter staff to foster a smooth transition for guests moving out of shelter and into permanent supportive housing.
• Collect and maintain data-based statistical information, maintain accurate case records in a timely fashion.
• Create and implement participant-centered individual service plans within required time frame with an emphasis on participant goals.
• Monitor participant progress and complete assessments to identify participant’s strengths, needs, barriers and goals in order to sustain housing and increase self-sufficiency.
• Locate and secure housing opportunities for participants often within a short window of identified time.
• Network and maintain positive working relationships with local landlords, property managers, housing authorities, social service organizations and state agencies.
• Provide support, facilitate crisis intervention, mediation and service coordination when necessary.
• Collaborate with internal and external partners.
• Participate in regularly scheduled supervision, agency and community meetings and trainings when required.
