Job summary

Father Bill's & MainSpring, based in Brockton, MA, seeks a Regular Full Time Hourly Case Manager to support families moving from homelessness to housing. The role requires a Bachelor’s degree or equivalent experience and CPR certification, with potential AED/First Aid as needed.

The CM will conduct intake, manage data, develop individual service plans, and coordinate with landlords and service providers. Local travel is required and the agency offers a benefits package.

Qualifications

• Bachelor's degree or equivalent work experience is required.
• CPR certification is required and provided by FBMS; AED/First Aid may also be required.
• Ability to complete intake within required timeframes and maintain data-based records.

Responsibilities

• Conduct intake for all family members 18+ within required time frames.
• Maintain data records and monthly/quarterly reporting as required by funders.
• Monitor progress and implement individual service plans for families 18+.
• Provide outreach and resource referral; assist with housing-related needs and landlord mediation.
• Conduct home visits and coordinate access to community resources; some transportation required.
• Attend supervision, meetings, trainings, and adhere to program funders' requirements.
