Job summary

Health Connect America in Alabama is seeking a Family Development Specialist to support children and families by developing treatment plans and linking them to services. This role is community-based, involving visits to clients' homes and collaboration with the treatment team.

You will provide individual, group, and family interventions, attend weekly team meetings, maintain compliant documentation, recruit referrals, and participate in ongoing training.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families, including internships/volunteer work.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, and skill-building as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families to services and resources identified in the treatment plan.
• Serve as a liaison with professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of all client interactions, treatment plans, and progress notes.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.
