Qualifications

• Education in human services or related field.
• Experience with people with disabilities preferred.
• Experience with person-centered planning preferred.

Responsibilities

• Advocates and coordinates services to meet client needs.
• Plans and updates Person Centered Plans of Service.
• Coordinates resources and referrals to achieve outcomes.
• Monitors progress and maintains HIPAA-compliant records.
• Represents SCI in a professional and confidential manner.
