Job summary

Support-Solutions- seeks a dedicated human services professional to perform case management, plan development, and service coordination for individuals with developmental disabilities. You will monitor services, document visits, and ensure compliance with authorization and billing requirements.

Travel to other company locations using a personal vehicle may be required; strong communication and organizational skills are essential for success in this role.

Qualifications

• Bachelor degree in a Human Services field as listed.
• Experience with developmental disabilities preferred.
• Proficient in Microsoft Office suite and basic office equipment.

Responsibilities

• Develops and updates Plan of Care and ensures timely service requests.
• Locates, coordinates, and monitors proposed waiver services.
• Provides case management and oversight of medical, social, and educational services.
• Facilitates crisis intervention and guides direct care staff.
• Documents visits and maintains case records with dates, locations, attendees, and summaries.
• Ensures prior authorization and proper service definitions before billing.
