Responsibilities include developing PCPs, coordinating services across providers, and ensuring compliance with state standards. Strong communication, documentation, and travel to client sites are required.

Qualifications

• Bachelor's degree required in a human services field or equivalent.
• Experience working with individuals with developmental disabilities is preferred.
• Knowledge of HIPAA and confidentiality practices is expected.

Responsibilities

• Advocate on behalf of individuals and educate families.
• Collect eligibility information and support documentation for services.
• Develop and monitor Person-Centered Plans (PCPs) and coordinate services.
• Travel to meet with clients and support staff to ensure service delivery.
• Adhere to SCI policies, participate in trainings and team meetings.
