Job summary

Lewis Mason Thurston Area Agency on Aging is seeking a dedicated Case Manager to assist functionally impaired adults in identifying services needed to maintain independence. The role requires interviewing clients, assessing needs, and coordinating supports in community settings.

You will develop and monitor individualized service plans, authorize services within guidelines, maintain records, and advocate for clients.

Qualifications

• Requires a master’s or bachelor’s degree in behavioral or health sciences and paid social services experience.

Responsibilities

• Performs comprehensive assessments via in-depth interviews and collateral contacts to evaluate client strengths, needs, and resources.
• Develops or modifies individualized service plans to meet client needs in a community setting.
• Implements service plans and coordinates with service providers and supports.
• Reassesses client situation and updates service plans within guidelines.
• Reauthorizes state core services and authorizes other services per guidelines.
• Provides client advocacy, referrals, consultation, and follow-up as needed.
• Documents case notes and maintains required forms and reports.
• Maintains knowledge of applicable laws and guidelines governing core services.

Education

Master’s degree in behavioral or health sciences

Bachelor’s degree in behavioral or health sciences

Bachelor’s degree and four years of paid social services experience
