Qualifications

Responsibilities

• Conducts comprehensive assessments using state-approved tools to evaluate client needs and resources.
• Develops or modifies individualized service plans to meet client needs in a community setting.
• Implements service plans and coordinates with service providers and informal supports.
• Reassesses client situations regularly to ensure plans remain appropriate.
• Reauthorizes state core services and authorizes additional services as allowed.
• Provides case management, advocacy, referrals, and crisis intervention when needed.
• Participates in case staffings and maintains required records and reports.
• Ensures compliance with laws, regulations, and agency policies.
• Participates in training and meetings as required.
