Qualifications

• Bachelor’s degree in a Human Services field or related field.
• Background check via fingerprinting and RAP Back enrollment.
• Ability to communicate with diverse clients, including those who are hard of hearing or cognitively impaired.

Responsibilities

• Provides information and assistance to clients seeking access to services.
• Screens and counsels on health insurance programs and coordinates with volunteers.
• Responds to inquiries by phone, in person, and walk-ins.
• Coordinates with Adult Protective Services and other agencies as needed.
• Provides community-based short-term case management and follow-up.
• Advocates for clients by linking them to services.
• Performs comprehensive assessments and annual reassessments for eligibility.
