NWSDS in Woodburn, OR is seeking a Case Manager to assess service needs, develop care plans, and coordinate supports for older adults and individuals with disabilities.

You will work closely with consumers, staff, providers, and community partners to connect people with in-home care, transportation, and community resources, while maintaining confidentiality and accurate records.

The role emphasizes person-centered service, strong documentation, and adherence to program requirements.

Competenze

• Bachelor’s degree in social sciences or related field
• 4+ years combined education/experience in social services or related work
• Experience working directly with consumers in social service settings
• Language proficiency test for bilingual positions (if applicable)
• Knowledge of social service programs and resources for older adults and individuals with disabilities
• Ability to document clearly and maintain accurate case records

• Meet with consumers to assess needs and identify support options
• Develop individualized care plans per program requirements
