Job summary

You will conduct assessments, develop individualized plans, arrange services, and maintain documentation while ensuring compliance with state regulations and agency policies. Some local travel is required.

Qualifications

• Master’s degree in behavioral or health sciences or equivalent.
• Bachelor’s degree in behavioral or health sciences with related experience.
• BA/BS with four years of paid social services experience acceptable.

Responsibilities

• Conduct comprehensive assessments via interviews and collateral contacts.
• Develops or modifies individualized service plans.
• Coordinate service arrangements with providers and supports.
• Reassess client needs within guidelines to update plans.
• Maintain case records and required forms per agency policy.
• Ensure compliance with applicable laws and regulations.
