State of Washington’s Aging and Adult Services of Coastal Washington (AASCW) in Long Beach, WA, seeks a Part-time Case Manager. The role focuses on assessment, care management and in-home services for eligible seniors and adults.

Work is 30 hours per week non-exempt with strong collaboration across agencies to support independence and quality of life. Qualifications require a Master’s Degree (or BA with related social service experience) and current WDL with insured vehicle.

Competenze

• Requires Master’s Degree or BA with related social service experience.
• Experience in case management or social services preferred.

Mansioni

• Provides assessment, reassessment and in-home care management services to frail and vulnerable adults who have been found eligible for Title XIX services by the Department of Social & Health Services.
• Collaborates with the client to create a service plan that maximizes independence and quality of life.
• Coordinates, updates and implements the client service plan within funding source requirements and agency benchmarks.
• Works with a multi-agency service delivery team to provide necessary services including advocacy under the Older Americans Act.

Formazione

Master’s Degree in behavioral or health sciences & 1 yr. paid social service experience or BA in similar fields and 2 yrs. paid social service experience; or BA and 4 yrs. paid social service experience
