Job summary

Area Agency on Aging & Disabilities of SW WA seeks a dedicated case management professional to assess needs, authorize in-home and community-based services, and help clients remain independent at home. You will develop and administer individualized service plans while coordinating with nursing staff and community providers.

Responsibilities include assessments, care planning, service authorization, and ensuring quality service delivery.

Qualifications

• Master’s degree in behavioral or health science with 1 year paid social service experience or
• Bachelor’s degree in behavioral or health sciences with 2 years paid social service experience or
• Bachelor’s Degree and four years paid social service experience or
• Relevant combination of education and experience considered.

Responsibilities

• Perform comprehensive functional assessments and reassessments for clients.
• Develop an appropriate plan of care to assist client with identified outcome objectives.
• Implement short- and long-term service plans including authorization of in-home and community services.
• Coordinate with providers of client services and maintain records.
• Coordinate services with other community service providers including mental health and disability services.
