Job summary

HomeWell Care Services is seeking a compassionate CNA or HHA to join the weekend 12-hour shift team in the Volusia, Flagler, and St. Johns counties area.

You will deliver direct patient care under nursing supervision, assist with ADLs, monitor vitals, help with transfers, and provide companionship. The role requires CNA/HHA certification, reliable transportation, and strong communication skills to coordinate with patients and healthcare professionals.

Qualifications

• Must have valid CNA or HHA certification or completed recognized training program.
• Minimum 2 years of caregiver experience preferred.
• Experience with dementia, ADLs/IADLs, wound care and catheter management.
• Ability to lift and transfer patients safely and follow instructions.

Responsibilities

• Provide direct patient care under supervision of a registered nurse or healthcare professional.
• Assist with activities of daily living (bathing, dressing, grooming, toileting).
• Monitor vital signs and document findings.
• Assist with mobility and transfers using proper techniques.
• Prepare meals per dietary restrictions and preferences.
• Provide companionship and emotional support to patients.
• Assist with light housekeeping tasks.

• Experience with Dementia

• Experience with ADL's and IADL's

• Experience with limited non weight bearing patients.

• Experience working with patients that has wound management

• Experience working with Super pubic catheters

• Experience with lift to pivot stands and transfers are needed

• RELIABLE and DEPENDABILITY IS A MUST!!!

• 2 YEARS MIN EXP WITH AS A CAREGIVER (preferred)

Responsibilities:

Requirements:

• Valid certification as a CNA or Home Health Aide or completion of a recognized training program.

• Experience in providing patient care preferred.

• Knowledge of medical terminology and basic understanding of healthcare practices.

• Ability to lift and transfer patients safely and comfortably.

• Excellent communication skills to effectively interact with patients, families, and healthcare professionals.

• Compassionate and caring attitude towards patients in need of assistance.

• Ability to work independently and follow instructions accurately.
