Job summary

HomeWell Care Services is seeking compassionate CNA/HHA caregivers to provide in-home care in the Volusia, Flagler, and St. Johns counties. The role focuses on dementia care, ADLs, wound management, and safe patient transfers, with reliable, compassionate service a must.

Responsibilities include assisting with daily living activities, vital signs, meals, companionship, and light housekeeping, under RN supervision as needed. Valid CNA/HHA certification required or training completed.

Qualifications

• Experience with dementia and ADLs/IADLs is preferred.
• Must have CNA or HHA certification or complete an approved training program.
• Ability to lift and transfer patients safely.
• Excellent communication with patients, families, and healthcare staff.

Responsibilities

• Provide direct patient care under supervision of a registered nurse or healthcare professional.
• Assist with activities of daily living (bathing, dressing, grooming, toileting).
• Monitor and record vital signs (BP, temp, pulse).
• Assist with mobility and transfers using proper lifting techniques.
• Prepare and serve meals per dietary restrictions and preferences.
• Provide companionship and emotional support to patients.
• Assist with light housekeeping tasks (laundry, tidying).

• Experience with Dementia
• Experience with ADL's and IADL's
• Experience with limited non weight bearing patients.
• Experience working with patients that has wound management
• Experience working with Super pubic catheters
• Experience with lift to pivot stands and transfers are needed
• RELIABLE and DEPENDABILITY IS A MUST!!!
• 2 YEARS MIN EXP WITH AS A CAREGIVER (preferred)

• Ability to lift and transfer patients safely and comfortably.
• Excellent communication skills to effectively interact with patients, families, and healthcare professionals.
• Compassionate and caring attitude towards patients in need of assistance.
• Ability to work independently and follow instructions accurately.
