Job summary

PACE of Southwest Michigan is seeking a dedicated Participant Care Assistant (CNA) in Niles, MI. The role focuses on providing compassionate care to participants enrolled in the PACE program, including support with daily living activities and comfort measures.

You will work closely with a skilled team in a 40-hour week, 8am–4:30pm, Monday through Friday, delivering patient-centered care and collaborating with the Day Center team to enhance participants' well-being.

Qualifications

• CNA certification is required.
• At least 1 year of experience with frail or elderly individuals.
• Valid Michigan Driver’s License with clean record.
• Current CPR or BLS certification or willingness to obtain.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Provide hands-on care (ADLs) such as bathing, grooming, dressing, and toileting.
• Support mealtime and help participants stay hydrated.
• Assist with mobility, transfers, and positioning.
• Measure and record vital signs, weight, and intake/output.
• Maintain clean, comfortable, and stocked living spaces.
• Respond with kindness and compassion.
• Report any changes in condition to leadership promptly.
• Maintain confidentiality and professionalism at all times.
