Lutheran Social Services of Wisconsin & Upper Michigan is seeking a Program Supervisor for the Children’s Long Term Support (CLTS) team in Ozaukee County. The role provides clinical and operational leadership for Care Coordinators and maintains regular contact with children and families served.

Key responsibilities include supervising staff, ensuring compliance with CLTS and Medicaid requirements, monitoring outcomes, and supporting program growth through community outreach and partner

Qualifications

• Bachelor’s degree in Human Services or closely related field.
• Minimum 2 years of experience working with children with disabilities and/or families.
• Valid driver’s license and reliable transportation.

Responsabilités

• Lead, coach, and supervise a team of Care Coordinators.
• Ensure quality service delivery and compliance with CLTS, Medicaid, licensing, and contract requirements.
• Monitor documentation, outcomes, and productivity standards.
• Conduct case reviews, case consultation, and provide direct service coverage as needed.
• Oversee staff training, performance evaluations, scheduling, and accountability.
• Partner with Human Capital on hiring, onboarding, and staff development.
• Support billing, financial oversight, and program budget management.
• Build and maintain relationships with families, community partners, and funders.
• Participate in program growth, marketing, and community outreach efforts.
