Lutheran Social Services of Wisconsin and Upper Michigan (LSS) seeks Care Coordinators to support children with developmental, physical, or emotional disabilities through the CLTS Waiver Program in Milwaukee County. You will serve as the primary contact for families, coordinating assessments, service planning, and access to community services.

Competenze

• Bachelor's degree in a health or human services field is required.
• Minimum of 1 year of experience working with children in the target population.
• Bilingual fluency in Spanish is preferred but not required.
• Valid driver’s license and reliable transportation are required.

• Conduct functional assessments and reassessments using approved tools.
• Develop individualized service plans with families and providers.
• Authorize and coordinate services in alignment with the plan.
• Facilitate family-centered team meetings.
• Maintain accurate documentation and client records.

• Testify at legal proceedings when required.
