Job summary

Lutheran Social Services of WI and Upper MI is hiring a Care Coordinator in Outagamie County, WI. The role supports families through the Children’s Long-Term Support (CLTS) Waiver Program, serving children with developmental, physical, or severe emotional disabilities.

You will assess needs, develop service plans with families and providers, and coordinate services while maintaining records. The position requires a bachelor’s degree in social work or related field and a valid driver’s license.

Qualifications

• Bachelor’s degree in Social Work or related Human Services field.
• Minimum 1 year working with children/youth with disabilities.
• Valid driver’s license and reliable transportation.
• Strong documentation and communication skills.
• Computer proficiency including electronic health records.

Responsibilities

• Assess functional abilities and needs using required tools.
• Develop and implement individualized service plans with families and providers.
• Coordinate and authorize services per plan.
• Facilitate family-centered team meetings.
• Maintain accurate documentation and client records.
• Apply trauma-informed, evidence-based practices.
• Participate in staff development and clinical supervision.
