Job summary

Lutheran Social Services of WI & Upper Michigan is seeking a full-time Care Coordinator to support children and families in Ozaukee County’s CLTS Waiver Program. You will assess needs, develop family-centered plans, and connect families with services while documenting progress and collaborating with partners.

This role offers a hybrid schedule with up to two remote days per week, standard Monday–Friday hours, and comprehensive health benefits. Equal opportunity employer committed to inclusion.

Qualifications

• Bachelor's degree in Social Work or a related Human Services field.
• At least one year of experience working with children and/or youth with disabilities, or an equivalent combination of education and experience
• Valid driver's license and reliable transportation
• Strong communication, organization, and documentation skills

Responsibilities

• Serve as the primary point of contact for children and families enrolled in the CLTS program
• Complete assessments and identify service needs
• Develop and coordinate individualized, family-centered service plans
• Connect families with community resources and providers
• Facilitate team meetings and collaborate with service partners
• Maintain accurate documentation and client records
• Provide trauma-informed, person-centered support
