Job summary

Life Navigators in Milwaukee is seeking a Children’s Long Term Support and Service Coordinator to manage a caseload of 40-45 children (0-18) under CLTS Waiver; the role emphasizes person-centered service coordination and transition planning.

The ideal candidate has a social work background (Bachelor’s) with strong communication skills and knowledge of public benefits; hybrid work setup is available to support work-life balance.

Qualifications

• Bachelor’s degree in Social Work, Public Health or related field; Master’s preferred.
• 1+ year of direct experience serving children, youth and individuals with disabilities.
• Knowledge of community-based services and public benefits programs.
• Excellent written and oral communication and detailed documentation.
• Knowledge of Milwaukee County service system and Wisconsin waiver programs.
• Proficient with Microsoft Office and SUMAC program management systems.
• Valid Wisconsin driver’s license, vehicle, and insurance.

Responsibilities

• Provide comprehensive assessment, evaluation and monitoring of services for CLTS participants; use a person-centered approach to ensure access and inclusion for home and community services.
• Ensure compliance with performance measurements including ISP outcomes, recertifications, required contacts, and incident reporting.
• Establish and monitor Individual Service Plans for children and families; review service plans and authorizations; coordinate transitions and cross-system coordination.
• Maintain certification as a Children’s Long-Term Care Functional Screener with the State of Wisconsin; participate in ongoing training.
• Provide coordination with families and service providers to promote safety, independence and community integration.
• Perform routine monitoring and crisis intervention; coordinate with health, behavioral and medical providers; timely reporting.
• Prepare written reports, correspondence and program documentation; complete case noting in SUMAC for reporting and contract compliance.
• Represent CLTS interests in community coalitions and advocate for children with disabilities and families.
• Administer policies and perform related duties as required.
