Job summary

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking CLTS Care Coordinators to support children with developmental, physical, or emotional disabilities in Milwaukee County. You will assess, plan, coordinate, and provide ongoing support to families in home and community settings.

The role requires collaboration with service teams, documentation, and crisis-responsive care. Remote work option: 2 days per week; daily travel and occasional overnight travel may be necessary.

Qualifications

• Bachelor’s degree in health or human services field.
• Minimum 1 year of experience with children in target population.
• Bilingual Spanish preferred but not required.
• Valid driver’s license and reliable transportation.
• Experience with electronic health records.

Responsibilities

• Perform functional assessments and reassessments.
• Develop individualized service plans with families and providers.
• Authorize and coordinate services per plan.
• Facilitate family-centered team meetings.
• Maintain accurate client records and documentation.
• Collaborate with teams and supervisors on interventions.
• Provide trauma-informed, family-centered services.
• Complete time reporting accurately.
