Qualifications

Position Summary

As a Clinical Support Coordinator with CVS Health, you will assist the Clinical Nurse Specialists with non-clinical duties related to coordinating home nursing for patients receiving specialty medications for chronic disorders in the home.

Responsibilities include, but are not limited to:

1. Coordinate home nursing for assigned specialty patients under the direction of a Nurse Specialist or team supervisor.
2. Identify nursing agency based on established guidelines, provide the nursing agency with clinical documents, nursing authorization and clinical competency link/test.
3. Management of information in multiple EMR systems, that includes but not limited to, documenting patient demographics, provider details, process / status notes, preparing and sending forms for signature, and discharging.
4. Assist the clinical nurse specialists with monthly nursing agency authorization renewals.
5. Follow-up with agencies to retrieve documents (competencies, signed nursing service agreements, license/certificate of insurance) and upload to CNS.
6. Assist clinical nurse specialists with Medicare/Medicaid agency searches.
7. Complete OIG/SAM searches to ensure agency compliance with federal programs.
8. Patient outreach to provide manufacturer injection training contact information.
9. Perform other duties as assigned.

** This is a full-time remote position.


Required Qualifications

  • Minimum one year of related experience; administrative support functions, and customer service.
  • Effective verbal and written communications skills.
  • Excellent organization and time management skills.
  • Ability to learn and work with multiple computer applications.
  • Proficiency with Microsoft business software (Outlook, Word, Excel).

Preferred Qualifications

  • Home Infusion, Home Health, or Medical Office experience
  • Coram Nursing Clinical Support experience


Education

High school diploma (required), Medical Assistant certification (preferred)

Pay Range

The typical pay range for this role is:

$17.00 - $25.65

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
