EventusWholeHealth is transforming healthcare delivery in skilled nursing and assisted living facilities through an integrated, evidence-based model. We seek a Clinician Support Specialist to coordinate workflows with APCs and facility staff, enhancing patient care, communication, and clinical services.

The CSS role combines clinical support, administrative coordination, and customer service in a fast-paced healthcare environment.

Competenze

High school diploma or equivalent.
Certified/Registered Medical Assistant, CNA, or PCT highly preferred.
Minimum one year of patient care experience highly preferred, medical scribing experience preferred.

Mansioni

Prepare for daily clinical rounds by ensuring patient information, documentation, and schedules are ready.
Assist with encounter documentation, data entry, and chart preparation.
Coordinate and maintain patient schedules, including admissions, discharges, and changes.
Serve as liaison between clinicians, facility staff, patients, and families to support communication.
Assist with triage needs and documentation as directed by the clinician.
Organize and manage clinical documentation, including labs and patient records.
Host and support telehealth visits for routine and acute care needs.
Obtain and process patient consents and route documentation.
Participate in facility meetings and communicate updates to the care team.
Support discharge planning and maintain accurate census and records.
Maintain professionalism and represent the organization positively.
