Job summary

Eventus WholeHealth is seeking a Clinician Support Specialist (CSS) to support the clinical care team across skilled nursing and assisted living facilities. You will coordinate workflows, assist with encounters, and enable effective communication among patients, providers, and care teams.

Responsibilities include triage support, documentation, scheduling, and telehealth, while travel to assigned facilities is required to ensure high-quality patient care and smooth operations.

Qualifications

• High school diploma or equivalent required.
• Certified/Registered Medical Assistant, CNA, or PCT preferred.
• Minimum one year of patient care experience preferred; medical scribing experience preferred.

Responsibilities

• Prepare for daily clinical rounds by ensuring all patient information, documentation, and schedules are ready.
• Assist clinicians with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain accurate patient schedules, including admissions, discharges, and changes.
• Serve as liaison between clinicians, facility staff, patients, and families to support communication and care.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation, including labs and patient records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and route documentation properly.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and ensure accurate census and records.
