Job summary

EventusWholeHealth is seeking a Clinician Support Specialist to back the clinical care team across skilled nursing and assisted living facilities. You will coordinate workflows with Advanced Practice Clinicians and facility staff, support encounter documentation, and help ensure efficient, high-quality patient care.

This role requires strong organizational skills, excellent communication, and willingness to travel regularly to assigned facilities.

Qualifications

• High school diploma or equivalent required.
• MA/CNA/PCT certification preferred.
• At least 1 year patient care experience preferred.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Prepare for daily clinical rounds by ensuring all necessary patient information, documentation, and schedules are ready.
• Support clinicians in delivering care by assisting with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain accurate patient schedules, including admissions, discharges, and acute changes.
• Serve as a liaison between clinicians, facility staff, patients, and families to support communication and quality care delivery.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation, including lab results, diagnostic data, and patient records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and ensure proper routing of documentation.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and ensure accurate patient census and records.
• Maintain professionalism and represent the organization positively within partner facilities.
