Job summary

EventusWholeHealth is expanding a clinician-facing role to support clinical teams in skilled nursing and assisted living facilities. The Clinician Support Specialist coordinates workflows, supports APCs and facility staff, and enhances communication among patients, providers, and care teams to improve care quality and efficiency.

The position emphasizes patient-centered coordination, documentation, and telehealth support with travel to assigned facilities.

Qualifications

• High school diploma or equivalent required.
• Certified/Registered Medical Assistant, CNA, or PCT preferred.
• Minimum 1 year patient care experience preferred; medical scribing experience preferred.
• Valid driver’s license and reliable transportation.
• Strong verbal, written, and electronic communication skills; organizational abilities in a clinical setting.

Responsibilities

• Prepare for daily clinical rounds with complete patient information, documentation, and schedules.
• Assist clinicians with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain patient schedules, admissions, discharges, and acute changes.
• Serve as liaison between clinicians, facility staff, patients, and families to support communication and care.
• Assist with triage and documentation as directed by the clinician.
• Organize and manage clinical documentation, including labs, diagnostics, and records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and route documentation accordingly.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and ensure accurate census and records.
