Ascension Health in Flint, Michigan is seeking a Full-Time daytime role at the PACE Clinic. The position focuses on coordinating participant appointments and services, including those outside the PACE center, and collaborating with external vendors and the care team.

Coordinate participant appointments and services, including care provided outside of the PACE center.
Work with external vendors, providers, and community partners to schedule and coordinate participant services.
Communicate with participants, families, and the care team to keep appointments and services on track.
Help ensure participants have access to the care, transportation, and support services they need.
Education:
High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
