Job summary

Ascension Living in Chattanooga, TN seeks a Registered Nurse to provide direct nursing care to PACE participants, perform acute triage, blood draws, wound care, and participate in after-hours on-call rotation.

You will work with the Interdisciplinary Team to develop and update individualized care plans, participate in care plan and family meetings, and travel to participant homes, long-term care, and assisted living facilities as needed.

Qualifications

• RN licensure/certification required prior to hire.
• BLS required within 30 days of hire date or job transfer date.
• Diploma from an accredited school/college of nursing or required professional licensure at time of hire.

Responsibilities

• Deliver high-quality nursing care to PACE participants, including acute triage, blood draws, and wound care; participate in after-hours/weekend on-call rotation.
• Collaborate with the Interdisciplinary Team (IDT) to develop, implement, and evaluate individualized care plans with timely updates.
• Participate in IDT care plan and participant/family meetings; perform assessments across care settings such as the PACE clinic, Adult Day Center, and residences.
• Educate participants and families on health management and end-of-life care; travel to homes, long-term care, and assisted living facilities as needed.

How you’ll make an impact in this role

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

Required Credential(s):

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What Additional Preferences We're Seeking

• One or more years of RN experience.
• Home Health experience a plus.
