Job summary

Ascension is seeking a licensed Registered Nurse to deliver high-quality nursing care to PACE participants in Chattanooga, TN, including triage, blood draws, wound care and after-hours on-call.

You will collaborate with the IDT to create and update care plans, participate in meetings, and assess care across the PACE clinic, Adult Day Center, and participant residences, with travel to homes as needed.

Qualifications

• Current RN licensure and BLS certification.
• Experience providing direct nursing care as an RN preferred.

Responsibilities

• Deliver high-quality nursing care to PACE participants, including triage, blood draws, wound care, and after-hours/weekend on-call rotations.
• Collaborate with the Interdisciplinary Team to develop and update individualized care plans.
• Participate in IDT care plan and participant/family meetings; assess across care settings (PACE clinic, Adult Day Center, residences).
• Educate participants and caregivers on health management and end-of-life care; travel to homes or facilities as needed.

How you’ll make an impact in this role

• Deliver high-quality, direct nursing care to PACE participants, including acute triage, blood draws, wound care, and participation in an after-hours/weekend on-call rotation.
• Collaborate with the Interdisciplinary Team (IDT) to develop, implement, and evaluate individualized care plans, ensuring timely updates based on participant needs.
• Participate in IDT care plan and participant/family meetings, and perform assessments across care settings such as the PACE clinic, Adult Day Center, and participant residences.
• Educate participants, caregivers, and staff on health management and end-of-life care, with flexibility to travel to homes, long-term care, and assisted living facilities as needed.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

Required Credential(s)

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date.

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What Additional Preferences We're Seeking

• One or more years of RN experience.
• Home Health experience a plus.
