Job summary

Ascension in Indianapolis, IN is seeking a full-time Home Care role to coordinate patient appointments, manage external referrals, and ensure accurate EHR documentation. The position supports primary care operations within the St.

• Scheduling & Referrals: Coordinate external medical appointments, referrals, and schedule changes for assigned patients.
• EHR Documentation: Log all appointments, special instructions, and patient details accurately into the electronic medical system.
• Provider Contact Management: Maintain and regularly update the external provider contact directory.
• Protocol & Compliance: Follow site-specific workflows and keep documentation up to date to ensure compliance.
