Job summary

Ascension Living is recruiting for a Full-time Home Care scheduling and coordination role in Indianapolis, IN. The position involves arranging appointments and referrals, documenting patient details in the EHR, and maintaining provider contact records to support compliant care delivery.

Major duties include coordinating referrals, updating records daily, and ensuring all workflows are followed. A high school equivalency or equivalent experience is required, with licensure substitution possible

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Responsibilities

• Coordinate external medical appointments, referrals, and schedule changes for assigned patients.
• Log all appointments, special instructions, and patient details into the EMR accurately.
• Maintain and regularly update the external provider contact directory.
• Follow site-specific workflows and keep documentation up to date to ensure compliance.

How you’ll make an impact in this role
