Job summary

Corewell Health in Taylor, MI is seeking a part-time PRN/Casual Social Work professional to collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs, including discharge planning and referrals.

You will conduct psychosocial assessments, coordinate with community resources, document in EMR, and provide support for bereavement and end-of-life decisions.

Qualifications

• Master's degree in Social Work required.
• Experience in assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• Michigan licensure on hire required.

Responsibilities

• Conduct psychosocial assessments with patients and families to support identified needs and discharge planning.
• Communicate with alternate sites of care and community agencies to address patient needs.
• Document assessments and plans in the EMR reflecting current condition and transition needs.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources and participate in committees/projects as assigned.
• Develop and maintain knowledge of federal/state regulations as they pertain to the role.

Education

Master of Social Work (MSW) – Michigan Licensure Required
