Job summary

MyMichigan Health System seeks a licensed social worker to deliver psychosocial-spiritual care across the continuum of patient care. You will assess needs, educate patients and families, and plan care with the healthcare team to improve outcomes and length of stay.

Qualifications

• MSW degree and licensure required; LMSW/ACS recommended.
• Three years hospital social work experience preferred.
• Experience with crisis intervention and discharge planning.

Responsibilities

• Complete and document psycho-social-spiritual assessments.
• Provide social work interventions for hospitalization and crisis.
• Participate in care coordination and daily meetings.
• Advocate and coordinate transitions to appropriate care levels.
