Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time, in the exam room and during follow-up visits. You will help with accurate and timely documentation to enable providers to focus on patient care.

The role is on-site with on-the-job training in value-based care and chronic disease documentation. A 1–2 year commitment is ideal, with strong listening and typing skills and a willingness to learn.

Qualifications

• Knowledge of medical terminology and coding concepts preferred.
• Prior clinical experience or shadowing is strongly preferred.
• Scribe or transcription experience is optional but beneficial.

Responsibilities

• Document patient encounters in structured notes including HPI, assessment, plan, and exam.
• Assign CPT and ICD-10 codes and prepare After Visit Summaries.
• Support clinical documentation improvement and liaise with the provider.
