Job summary

Your Health in Georgia is seeking a Clinical Scheduler to support our Care Manager role, coordinating all in-person visits for a single interdisciplinary care team across the Blue Ridge area. The scheduler ensures proper number and type of encounters under our proactive, value-based care model.

This full-time, 8-hour position is in the office daily (8:00 AM–5:00 PM, Monday–Friday) and emphasizes scheduling accuracy, timely communications with patients, providers, and facilities, along with

Qualifications

• High school diploma or equivalent required; Associate's or Bachelor's in Healthcare preferred.
• Preferred CMA, LPN or RN credentials.
• Experience in medical/healthcare settings, preferably scheduling or administrative.
• Familiar with medical terminology, procedures, and insurance verification.

Responsibilities

• Register, verify and schedule patients according to the care model.
• Track and follow up with new patients until scheduled.
• Schedule appointments based on care model, availability and urgency.
• Optimize all disciplines' schedules.
• Follow patients after discharge and ensure timely appointments.
• Schedule timely TCM and other required visits within the window.
• Maintain strong documentation.
• Operate across multiple software systems to coordinate follow-ups.
• Interact with patients via email, patient portal, telephone and in person to gather information and confirm appointments.
