Qualifications

• High school diploma or equivalent required; healthcare education preferred.
• Experience scheduling in medical/healthcare settings.
• Familiarity with medical terminology and insurance verification.

Responsibilities

• Register, verify and schedule patients according to the care model.
• Track new patients and ensure timely scheduling.
• Schedule appointments based on availability, urgency and patient preferences.
• Coordinate disciplines to optimize schedules.
• Follow patients after discharge and schedule subsequent visits.
• Schedule timely Targeted Care Management (TCM) appointments.
• Maintain thorough documentation in scheduling systems.
• Use multiple software and tracking systems to support follow-up.
