Your Health Organization in Greenville is seeking a Clinical Scheduler to support Care Manager operations for Belton/Pelzer/Williamston/Piedmont patients. The role serves as the central scheduling and operational coordinator for a single interdisciplinary care team, ensuring patients receive the appropriate number and type of in-person encounters under our value-based care model.

This full-time, salary-based 8-hr position (8:00AM-5:00PM) (Monday-Friday) will work at the Greenville office daily,

Formación

• High school diploma or equivalent required; healthcare degree preferred.
• Preferred CMA, LPN or RN.
• Experience in a medical or healthcare setting, preferably scheduling.
• Familiar with medical terminology, procedures, and insurance verification.

Responsabilidades

• Register, verify and timely schedule patients according to the Company’s care model.
• Ensure new patients are being tracked and followed until scheduled.
• Schedule appointments for patients according to care model including follow-ups with healthcare providers based on availability, medical urgency, and patient preferences.
• Ensure all disciplines with schedules are fully optimized.
• Ensure patients are being followed and scheduled appropriately when discharged from an in-patient facility.
• Ensure TCM appointments are scheduled promptly and within the required window.
• Ensure strong documentation is implemented.
• Interact with patients via email, patient portal, telephone, AthenaText, in-person, etc. to gather necessary information, schedule and confirm appointments, and provide pre-appointment instructions.
