Job summary

Your Health in Greenville is seeking a Clinical Scheduler to coordinate all in-person visits for our care team across Belton, Pelzer, Williamston, and Piedmont patients. This full-time role ensures the right number and type of encounters under our value-based care model.

Based in the Greenville office, you will manage scheduling, patient communications, and care team coordination to support proactive monitoring and improved outcomes.

Qualifications

• High school diploma or equivalent; associate or bachelor's in healthcare preferred.
• Preferred CMA, LPN or RN.
• Previous experience in medical scheduling or administrative role.
• Familiar with medical terminology, procedures, and insurance verification.
• Strong organizational and time management skills; multitask effectively.
• Excellent communication and interpersonal skills; build relationships.
• Ability to work under pressure.

Responsibilities

• Register and timely schedule patients per the care model.
• Track new patients and ensure they are scheduled.
• Schedule appointments based on availability and urgency.
• Optimize all disciplines' schedules.
• Follow patients after discharge and schedule accordingly.
• Schedule timely TCM appointments within required windows.
• Maintain strong documentation and use multiple software systems.
• Interact with patients via email, portal, phone, and in person to gather info and provide instructions.
