Qualifications

Responsibilities

• Interpret medication orders for safety and proper use; coordinate problem-solving among pharmacy, nursing and prescribers.
• Monitor therapy for drug interactions and therapeutic duplications; intervene when necessary.
• Evaluate medication choices for therapeutic outcomes and communicate issues to prescribers.
• Provide pharmacokinetics expertise to clinicians and participate in therapeutic drug monitoring programs.
• Document clinical activities and maintain accurate records in the patient chart and formulary.
• Oversee dispensing processes, including sterile products and chemotherapy.
• Educate staff and patients about medications and respond to emergencies as needed.
