Qualifications

• Active Illinois Pharmacist license (IDFPR) required.
• Pharmacy degree (B.Pharm or PharmD) required.

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications; coordinates problem solving between pharmacy, nursing and prescribers.
• Monitors drug interactions, therapeutic duplications and adverse effects; intervenes as needed.
• Maintains comprehensive patient pharmacotherapy records and ED/EMR documentation.
• Educates patients and health professionals about medications and therapy goals.

Bachelor’s Degree in Pharmacy or Doctor of Pharmacy Degree
