Job summary

Northwestern Medicine in Illinois seeks a Clinical Pharmacist to ensure safe, effective medication use and provide expert information to clinicians and patients within a comprehensive pharmacy services system.

You will interpret orders, monitor drug interactions, collaborate with nursing and prescribers, document activities, educate staff and patients, and participate in quality improvement initiatives to support patient-centered care.

Qualifications

• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree.
• Active Clinical Pharmacist license in the state of Illinois (IDFPR).
• Preferred: training and knowledge of specialty areas (neonatology, critical care, oncology, etc.) or completion of a residency program.

Responsibilities

• Interprets medication orders to ensure safety and proper use; coordinates issue resolution between pharmacy, nursing and prescribers.
• Monitors therapy for significant drug interactions or therapeutic duplications; intervenes when warranted.
• Evaluates appropriateness of medications and communicates potential problems to prescribers.
• Practices pharmacokinetics and adverse reaction consulting with physicians and nurses; participates in therapeutic drug monitoring.
• Documents all clinical activities and interventions; maintains patient profiles and accurate EHR records.
