Northwestern Medicine is seeking a Clinical Pharmacist to identify and resolve medication problems, provide authoritative information to health care professionals and patients, and ensure pharmacy services integrate to deliver patient-centered care.

The role includes monitoring therapy, recommending changes, documenting activities, educating staff and patients, and participating in quality improvement to advance safe medication use across the hospital.

Qualifications

• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy required.
• Active Clinical Pharmacist license in Illinois (IDFPR) required or preferred.

Responsabilités

• Interprets medication orders to ensure safety and proper use of medications.
• Monitors medication therapy for significant interactions and intervenes as needed.
• Evaluates and documents that medications are appropriate for therapeutic outcomes.
• Communicates potential problems and solutions to prescribers.
• Processes medication order information into the Electronic Health Record.
• Maintains comprehensive patient profiles and monitors inventory and dispensing processes.
• Compounds and dispenses pharmaceuticals including sterile products and chemotherapy.
