Northwestern Medicine is seeking a Clinical Pharmacist to ensure safe and effective medication use within the hospital system. You will interpret orders, monitor therapy, and coordinate with physicians, nurses, and pharmacists to optimize patient outcomes.

Responsibilities include drug information provision, pharmacokinetic consultation, and accurate documentation in the electronic health record. The role supports therapy monitoring, formulary management, and education across teams.

• Acts as a consultant in pharmacokinetics and adverse reactions to physicians, nurses and other healthcare professionals.
• Processes all medication order information into the Electronic Health Record to ensure accuracy of records.
• Maintains a comprehensive pharmacy patient profile with key patient data.
• Oversees packaging, labeling, and distribution of medications per policies; monitors controlled inventories.
• Compounds and dispenses pharmaceuticals including sterile products, chemotherapy and parenteral nutrition.
• Verifies medication orders before delivery to patient care areas and resolves discrepancies.
